To help ensure the highest quality of service and care to our patients, we have several office policies and procedures. If you have any questions or concerns with these procedures, please address them with the staff prior to your office visit.
What to Bring: We require that you always bring the following to your office visit:
Cancellations/No Shows: We realize that your schedule may unexpectedly change. If you need to cancel your appointment, please notify us as soon as possible, preferably 24 hours in advance. Anything less than 24 hours notice does not allow us enough time to ensure another patient can be scheduled.
Please be advised that there may be times when due to medical emergencies, we may have to reschedule your appointment. We apologize in advance for this inconvenience.
What do I do if I need prescription refills?
We will be pleased to call to renew or refill prescriptions at your pharmacy. We ask that you tell us the name and dosage of your medication and the name and phone number (and the fax number if possible) of your pharmacy. You may also email us at prescriptions@likhc.com. Please, in all communications include your first name, last name, middle initial, date of birth, phone number and email address. Please contact us a few days prior to running out of the medication. If you need immediate refill, please call us (rather than email us) so that we can ensure that there is no disruption in your medication.
What hospitals are you affiliated with?
North Shore University Hospital, Manhasset; St. Francis Hospital, Roslyn; North Shore University Hospital, Glen Cove and Long Island Jewish Medical Center.
What insurance plans does the office accept?
We accept most insurance plans. To be absolutely sure, you can call our office or call your insurance company directly and ask if we are participating providers.

How do I reach you for a general question (Non-emergency Emails)?
We realize the value of using email as a tool of communication and we encourage your use of email for non-emergency questions. Questions of a more critical nature should still be handled by the telephone. In the case of an actual life-threatening emergency, please dial 911. Through email we can provide information about our practice including information about appointments, office hours, etc. We will attempt to answer emails as soon as possible with most emails answered in 24-72 hours.
